🩺 Comprehensive Services Designed for You
Through this partnership, individuals will have direct access to a wide range of essential health and wellness services, including:
Primary & Preventive Care: Family medicine, routine wellness checks, chronic disease management (including diabetes screening/education and blood pressure management), and women’s health services.
Sexual Health & Wellness: Comprehensive sexual health clinic services, STI/HIV screening, prevention education, and PrEP services (offered both in person or via telehealth/virtual appointments).
Mental & Behavioral Health: Dedicated behavioral health support and counseling.
Holistic Support: Social Determinants of Health (SDOH) screenings to identify unmet needs, connect individuals with community resources, and provide dedicated care coordination and referrals.
