Through this partnership, individuals will have direct access to a wide range of essential health and wellness services, including:

Primary & Preventive Care: Family medicine, routine wellness checks, chronic disease management (including diabetes screening/education and blood pressure management), and women’s health services.

Sexual Health & Wellness: Comprehensive sexual health clinic services, STI/HIV screening, prevention education, and PrEP services (offered both in person or via telehealth/virtual appointments).

Mental & Behavioral Health: Dedicated behavioral health support and counseling.

Holistic Support: Social Determinants of Health (SDOH) screenings to identify unmet needs, connect individuals with community resources, and provide dedicated care coordination and referrals.

We are deeply committed to ensuring every individual has access to care that meets their unique needs. Beyond our standard services, we provide seamless care navigation and connections to trusted providers for gender-affirming hormone therapy and gender-affirming surgical services.
🗺️ Open to All of Western New York (WNY)
While this partnership is physically located in Niagara Falls, these services are open to anyone across the entire Western New York region. Whether you live in Erie County, Orleans County, or beyond, you are welcome here. We know that finding safe, affirming healthcare can be a challenge, and we are proud to serve as a regional resource. There are no residency restrictions—if you can make it to our Mobile Health Unit, our doors are open to you.
